THIS NOTICE DESCRIBES HOW WE HANDLE AND MAINTAIN CONFIDENTIAL PERSONAL FINANCIAL INFORMATION YOU PROVIDE TO US. PLEASE REVIEW THIS NOTICE CAREFULLY.
A. PRIVACY POLICY SUMMARY
We are very aware of your concerns about the privacy of non-public personal and financial information.
Keeping your personal and financial information secure is one of our most important responsibilities. We regularly review our information security standards and practices to protect against unauthorized access to information about our employees, members, visitors, vendors and others. Our staff are required to treat all personal and financial information of our employees, members, visitors, vendors and others as confidential and our staff are subject to disciplinary action if they fail to observe these provisions.
Full Gospel Holy Temple Church-Houston (“The Church”) does not sell or share your information with third
party marketers and others. Because of this policy, there is no need for you to request or “opt -out” of any personal or financial information sharing. Should we change that policy, we will provide you with a prior notice and opportunity to “opt-out” at such time. This Notice is intended to provide you with The Church’s policy on the following:
• what information we collect and how we use it;
• our obligations with respect to the disclosure of your personal and financial information; and
• your privacy rights in your personal and financial information.
B. COLLECTION AND USE
We collect information about you from various sources, including (1) your membership application and
the personal and financial information you provide to us in such application, such as your name, address, telephone numbers, social security numbers, bank accounts, assets, etc., and( 2) If you donate to the church and use a credit card, we will also request the credit card number and other information necessary to process payment in such manner (this also applies to on-line giving as well).
We use the information we collect to distribute year end giving statements, W2’s, 1099’s, etc. as statutorily required by law.
C. DISCLOSURE
We do not disclose your information to third party marketers. We do, however, disclose the personal and private information we have collected about you to third party service providers and the like, as permitted by law, who perform services on our behalf and assist us in processing financial transactions. We also may need to share your information if required by law-for example, in response to a subpoena from or an audit by a state or federal government agency.
D. YOUR RIGHTS
Ordinarily, you have the right to “opt-out” of allowing The Church to provide your personal or financial information to unaffiliated third parties by contacting The Church and asking that it not provide such information to unaffiliated third parties. However, The Church already has a policy that it will not do so. Accordingly, you do not need to contact us for such purpose. If The Church should desire to amend this policy, it will contact you prior to doing so and provide you the opportunity to opt-out at such time.
You have the right to review the personal and private information you provide to The Church, and the information we collect to be sure that it is accurate and up-to-date. You can obtain a copy of your information, including your contribution statement, at anytime by contacting The Church at the telephone number and address set forth at the end of this Notice. If the information is incomplete, inaccurate or not correct, please call or write The Church as noted. The Church will promptly update or correct any erroneous information.
